Healthcare reform mandates that most US citizens and legal residents purchase “minimal essential coverage” for themselves and their dependents, either through their employers or through marketplace “exchanges”. There is a financial penalty if an individual does not purchase.

He or she would have to pay whichever is greater: $695 for each uninsured family member, up to a maximum of $2,085; or 2.5 percent of household income. However, the individual mandate is supported by federal subsidies which support lower and middle classes.

Beginning in 2014, people who are unemployed, self-employed, or work for businesses that don’t offer insurance are able to shop for coverage in health exchanges. About 25 million people are expected to shop for coverage in exchanges. And of these 25 million, about 19 million are predicted to be eligible for aid. The cutoff point would be an income of four times the federal poverty level. For an individual, that’s about $44,000 a year. For a family of four, the figure is about $88,000.
Subsidies will be figured on a sliding scale; people who make three or four times the poverty level would get enough federal money so that they would pay no more than about 10 percent of their income for health insurance. The federal subsidy would go straight to the insurer; it would look like a discount on the policy to the customer.

Changes to Medicare are included in the plan. The plan does not contain cuts to traditional Medicare benefits, although Medicare payments for some services, such as home healthcare will be reduced by $40 billion between now and 2019. And some payments to hospitals will be cut by $22 billion over that same period.

Government payments to Medicare Advantage – plans that are run by private insurers and are an alternative to traditional Medicare – will be cut by $132 billion over 10 years. Medicare Advantage plans often offer extra benefits that seniors in traditional Medicare don’t receive. These extras may be dropped as Medicare Advantage plans experience budget cuts.

The bill would bolster the existing Medicare prescription-drug benefit by addressing gaps in coverage. Currently, after a beneficiary has spent $2,700 on drugs in a year, coverage stops until that same beneficiary has spent $6,154, when coverage is then reinstated.

Starting this year, seniors who are in between benefits will get $250 towards prescription drug costs from the government. Thereafter, Medicare will gradually increase the percentage of drug costs it pays within this gap. By 2020, the government will pay 75% of senior drug costs between $2,700 and $6,154.

Medicare will also begin to cover for annual wellness visits.

Employers with 50 or fewer workers would be exempt from coverage provisions. Firms with 50 workers or more may be subject to fees if healthcare insurance is not offered. And if you do offer coverage and you have low or middle income workers, there may be an additional expense.

If you are a firm with more than 50 employees, and do not offer health insurance as a benefit, and at least one of your full-time employees gets a subsidy from the federal government to purchase health insurance on his or her own, you would have to pay a fee of $2,000 for every one of your full-time workers. However, you can subtract the first 30 of your employees from that assessment.

Even if you do offer coverage, you might have to take some extra action to help any of your low or middle-income workers who want to buy insurance on their own. Low and middle-income workers are defined as an employee who makes less than 400% of the federal poverty level, which today is about $10,800 for an individual or $22,000 for a family of four.

If it is the case that the employee finds the company-offered insurance expensive, and their share of health premiums is more than 8% of their income (but less than 9.8%), they have the option of purchasing their own insurance through the “exchange” marketplaces that the new bill establishes.

The employer would have to provide the employee a voucher equal to what the company would have paid to provide coverage within the company plan.

Firms with more than 200 employees that offer health insurance must automatically enroll workers in the plan. Employees can opt out, but they have to make that decision.
